Palomar UC San Diego Health has partnered with Sharecare to fulfill your request for medical records.

Request Your Medical Records Online

  • Patients: Click on the “Begin” button below and fill out the form to request your medical records. You will be asked to provide a copy of your ID as part of the process.
  • Third-Party Request: Click on the “Begin” button below, fill out the requested details and upload your request.
  • Note: You may be contacted to provide additional documentation, such as a birth certificate, power of attorney, etc.
  • You will receive an update or response from Sharecare within 3 business days.

Patient Request

This is for patients requesting a copy of their own medical records or legal guardians or patient representatives with a power of attorney.

Third-Party Request

This is for providers, law offices or if you are requesting the medical records of someone other than yourself and are not the legal guardian.

Subpoenas for Medical Records, Billing Records and Radiology Images

Subpoenas must be addressed to the location name and address from which you are requesting records. Subpoenas should not be addressed to Sharecare. Please see a list of the acceptable locations below.

  • Palomar Medical Center Escondido: 2185 Citracado Parkway, Escondido, CA 92029
  • Palomar Medical Center Poway: 15615 Pomerado Road, Poway, CA 92064
  • The Villas at Poway: 15615 Pomerado Road, Poway, CA 92064 (Subpoenas will not be accepted at this location)
